Register Month-To-Month

To register for a our three-month testimonial program and month-to-month classes or if you’re an ICON employee please fill out your name, email address and contact phone number in the Google spreadsheet associated with the class of your choice. We also ask you specify if you’re signing up for our testimonial program or our month-to-month classes. If all 18 spots are full please find a class that will work with your schedule that isn’t full. Please fill out an empty line and DO NOT alter others information.
